A psychiatrist is forced to treat a patient who may pose a real threat, all while being bound by confidentiality. That creates a situation where every decision can lead to something more, leaving risk at every turn. The film understands that from the start and doesn’t waste time getting into it.

Michael Cerveris anchors the story by keeping everything on the surface under control. His take on Dr. Stuart Prince isn’t showy. He plays it like someone who’s always thinking a few steps ahead, trying to manage risk in real time. That simple thing really works in the film’s favor. Instead of showing fear, he lets it sit just below the surface, which makes the pressure feel ever-present throughout the film. You can see the moment-to-moment calculations happening, especially once it becomes clear that the situation isn’t contained.

David Conrad works well opposite him by avoiding anything too obvious. His performance doesn’t rely on big swings in the way that you would expect from a character like this. There’s a level of control that makes it harder to pin down what his character is capable of, and that uncertainty keeps their interactions on edge with a depth of anxiety. The film doesn’t hand the audience simple answers in those exchanges, which helps maintain interest when the story slows.

The strongest element here is the ethical conflict. The film keeps returning to the same question without resolving it. At what point does obligation stop being something you uphold and start becoming something that puts you at risk? That drives most of the film's decisions and gives the story an identity. There’s something undeniable about the drive and dedication here.

The setting is so important to this film; using Pittsburgh as a backdrop gives it a sense of place without drawing attention to itself. Offices, streets, and everyday locations feel familiar, which makes the situation feel more as if this is the real world and not a set. That choice works because the story doesn’t need heightened visuals to sell the danger. It relies on the idea that this could happen in a normal world.

Where the film starts to lose focus is in how it expands beyond that conflict. The main idea is strong enough to carry the runtime, but the script adds depth that the film doesn’t always take the time to dive into or explore as it should. Instead of tightening the focus, those elements occasionally divert attention from the core relationship between doctor and patient. The result is a narrative that feels less direct than it should.

The dialogue follows a similar pattern. When the film stays inside the therapy sessions, it’s sharp and deliberate with a story to tell. Those scenes carry the most because every line feels like it matters. Outside of that space, the writing leans more on explanation. It starts to say things outright that would have been stronger if left implied, which takes away some of the tension the film builds elsewhere.

The pacing sits in an uneven middle ground. It never stalls out at a middle point, but it doesn’t push forward when it should. There are stretches where it feels like the film is waiting for something to come to it instead of moving toward it. Once it commits, especially in the later portion, it becomes more engaging, but getting there takes longer than necessary.

The ending is where the film pulls things together in a way that really shocked me. It doesn’t rely solely on some major twist. Instead, it reframes earlier moments in a way that makes you reconsider what you’ve seen. It rewards paying attention without feeling like it’s trying to trick the audience, and that goes a long way in leaving a stronger impression than the first two acts might suggest.

Director Melissa Martin keeps a focus on character rather than chaos. The film doesn’t try to inflate itself beyond what it is. It stays centered on choices and consequences, even when the structure around them wavers. What holds BASIC PSYCH together is its commitment. It doesn’t simplify the situation or offer a resolution to the ethical questions it raises. Even when the narrative struggles, that core conflict keeps it from drifting too far.

It ends up in a space where it’s watchable from start to finish, with moments that stand out, but not quite as tight as it could be. The performances and the final act carry it. There’s a strong film in here, and you can see it at times, but it doesn’t stay locked into that version from start to finish.

Please visit https://linktr.ee/overlyhonestr for more reviews.

You can follow me on Letterboxd, Instagram, Twitter, and YouTube.
